Tax Appointment - What to Bring

Individuals
  • Last year's tax return (if we did not prepare it) 
  • Estimated payments - date of payment and amount (cancelled check)
  • W-2 Wage Statements
  • 1099's Interest and Dividends, from banks and financial institutions
  • Refunds from State Departments (e.g. Massachusetts state tax refund)
  • Pension distributions, including Social Security and Social Security Disability Income (1099-R, 1099-SSA)
  • Unemployment compensation (1099-G)
  • Information on any sale (stocks, mutual funds, personal property, investment properties-- Include purchase date, purchase price and any additions to basis)
  • Real Estate taxes
  • Personal property taxes (cars, boats, motor homes, airplanes, etc.)
  • Home mortgage interest (include interest on Home Equity Lines of Credit) 1098-Mort. Int.
  • Charitable gifts, both cash and non-cash
  • Unreimbursed employee expenses
  • Escrow papers (if you refinanced, sold or bought a home during the year) -- Settlement Statement
  • If you have your own business, all business income and expenses (see Sole Proprietor)
  • Rental expenses from home or other property that you rent to someone else
Sole Proprietor & Single Member LLC 
To save you time and money on your tax return, please allow us to prepare your business QuickBooks®.  If you use another accounting/bookkeeping service, please follow the guidelines below:
  • Reconcile all your checkbooks, savings accounts and credit cards before sending us your QuickBooks® file.
If you have paid for expenses personally, and they are NOT reflected in QuickBooks®, please give us a list including the date of purchase, what was purchased and how much it costs.  If you use a personal vehicle for your business, or if you have a business car that has any personal use, we need:
  • Make, model and cost of the vehicle
  • Date first placed in business use
  • Total mileage driven during the year
  • Total business miles driven
  • Distance to your place of work, and the total number of commuting miles
  • Total personal miles
  • Separate listings for each business use vehicle
It is helpful to have copies of:
  • All credit card statements 
  • All bank statements 
  • All line of credit statements
  • Any loan statements 
  • All 1099-Misc., reflecting Income Earned
  • Send copies of all new lease agreements that you entered into during the year
  • Payroll reports (all quarters), if we didn't do the returns.
  • December Sales Use Tax report
  • Your QuickBooks® back-up file. (.qbb file) Please do not send an accountant's copy or self burned CD (hard copy preferred)
Corporation / Partnership
To save time and money on your return, allow us to prepare your business QuickBooks®.  If you use another accounting/bookkeeping service, please follow the guidelines below:
  • Reconcile all your bank and savings accounts before sending us your QuickBooks® file.
  • Reconcile all your credit cards before you send us your QuickBooks® file.
If you have a business car that has any personal use - we need:

  • Total business miles
  • Total personal miles
It is helpful to have copies of:
  • All credit card statements 
  • All bank statements
  • All line of credit statements 
  • Any loan statement
  • All new lease agreements that you entered into during the year
  • Copy of your corporation's annual minutes
  • Payroll reports (all quarters), if we didn't do the returns
  • December sales tax report
  • Your QuickBooks® back up file. (.qbb file) Please do not send an accountant's copy or self burned CD (hard copies preferred)
  • Last year's tax returns (if we did not prepare it)
  • Articles of Incorporation or Organization
  • If an LLC or Partnership - Operating Agreement
  • Names and Social Security Numbers for all Members, Partners etc.
  • % Ownership, Contributing Capital etc. for all Members and/or Partners (M1 & M2 Reconciliation, Basis Reports)
